I, the undersigned, hereby authorize NET Microfinance Bank (NET MFB) to directly debit my salary from the source (salary account as provided in the form) for the purpose of repaying my loan as per the details provided above. I understand and agree to the following terms:

NET MFB: I consent to NET Microfinance Limited obtaining information from relevant third parties as may be necessary, on my employment details, salary payment history, loans and other related data, to make a decision on my loan application. I also consent to the loan amounts being deducted from my salary at source before credit to my account; and any outstanding loans being recovered automatically from any BVN accounts linked to me in the case of default

1. Mandate Setup: I hereby consent to NET MFB setting up a mandate with my employer to facilitate the direct debit of my salary for loan repayment purposes. I authorize NET MFB to liaise with my employer to establish and maintain this mandate.
2. Authorization: I authorize my employer or remita to deduct the specified repayment amount from my salary and remit it directly to NET MFB as per the agreed schedule.
3. Duration: This authorization will remain in effect until the loan is fully repaid or until I provide written notice to NET MFB and my employer to terminate this consent.
4. Changes: I agree to notify NET MFB promptly of any changes to my employment status or banking information that may affect the direct debit arrangement.
5. Termination: I understand that terminating this consent does not relieve me of my obligation to repay the loan in full according to the terms of the loan agreement.
6. Liability: I agree that neither my employer nor NET MFB will be held liable for any errors or delays in the processing of the direct debit payments.

This consent form is governed by the laws of Nigeria

• Any disputes arising from this consent form shall be resolved in accordance with the dispute resolution mechanisms set forth in the loan agreement.
